it looks like the box to the game Operation, which I agree looks like it points to TSM.

D'oh, I am too late... I am 99% sure Dr. Ravenscroft is right. It looks to me like the outer part of Test Track in Tomorrowland in Epcot.

ok, Dr. Ravenscroft got the last one right, but he's having some computer troubles, so I'll go ahead and post the next pic. hopefully this'll last more than a couple hours...
