What's even worse is when I give them the money for the bill and they ask me if I want change. Of course I want my change. It's mine until such time as I decide to give it to someone else. If the waiter asks if I want my change that automatically lowers whatever tip they might have gotten.
If someone added a 'suggested tip' on my bill (if I didn't have a large party), I'd be annoyed. That'd almost be a good way to guarantee I leave something other than that amount. I
might be willing to be lenient on a place like WDW (or any big tourist attraction place) for doing it though, since they have people from all over the world and tipping is not a universally standardized thing. It's a tricky line between making sure the wait staff get what they need for their income (assuming they make min. wage or lower) and insulting the guests, I think.
I can understand the 'do you need change?' a little better. Lots of wait staff don't count the money in the bill holder right when they pick it up, and some people do include the tip in the whole shebang when they pay cash. So I think it's faster for them to ask 'do you need change?', meaning 'should I go count this and ring it in right away and come back so you can leave, or can I take those people over there their sodas first because you're done and are leaving now?'.
There are definitely servers who ask it rudely though, and I'm sure they don't all deserve the benefit of the doubt I'm giving there. The few times I've had to wave people down to get them to bring me my change after they've assumed I didn't want it have definitely resulted in no tip for them.
