What I've heard is the answer to both questions of how the countries where chosen and why they don't build more come down to the same thing. Sponsorship! I doubt if a large company came up to Disney and said "hi, we want to build a copy of Petronas Towers for a Malaysia pavilion." They would turn them down.

Finally I've heard in various interviews that there seems feelings are that MK and Epcot are "mature parks" and they don't have to do much to keep them going. Right now the effort is being concentrated on AK.
As far as Africa? Well it's pretty well represented considering there is the Africa "outpost" (Central/South Africa) and Morocco (North Africa) add to that they then there is Africa at AK.
